I James Lee do solomnly and sincerely declare that I was enlisted on the 21st July 1899 under the name of William Heaps, which name I now declare to be incorrect. The name of James Lee I now declare to be my true name and I make this solemn declaration, consoientiously believing the same to be true, and by virtue of the provisions of an act made and passed in the sixth year of the reign of King William the Fourth, chapter 67, entitled the Statutory Declaration Act 1835.
